As Family Physicians, we have a duty to support and care for our individual patients. We also have a duty to go beyond: to act and advocate for the broad social determinants of health that affect the lives of the patients we see every day. We have the privilege and opportunity to help create transformative, meaningful, community led partnerships which can influence broader policy and practice. Through this, we can hope to truly influence the determinants of sickness and health of everyone, but specifically of those who struggle the most, or are furthest from care, in the communities of which we are a part.
